    Abstract

    The chapter provided an overview of the establishment of a student-lead wellness program, which focussed on falls, cognition and continence, for residents of an aged care village. Student participants were from the disciplines of pharmacy, nursing and physiotherapy. Collectively the students undertook a review of the residents, and devised a plan to address the three areas outlined above. the program was overseen by an accredited pharmacist who completed a home medicines review based on the students report and liaised with the residents general practitioner. The chapter also provides a summary of the stakeholders evaluation of the program, and the students perceptions on how the program had influenced on their preparedness for interprofessional practice
